Hazel I. Cook

COOK, Hazel I. Age 102, passed away Sunday, August 7, 2011 at Heartland of Miamisburg Nursing Home. Hazel was born July 12, 1909 to Roy & Stella (Libecap) Vandavere and was a resident of Farmersville the majority of her life. She was also a member of the Farmersville United Church of Christ, the Farmersville Historical Society, and New Lebanon Nutrition Center. Preceded in death by husband, Everett Cook; daughter, Joyce Garrett; son, Jerry Cook; special neice and nephew, Kathleen & Frank Weidner of Germantown. Survived by a daughter-in-law, Glenda Cook; 5 grandchildren, Lee Ann (John Grissinger) Breeding of Centerville, Kelly & Wendy Cook of Dallas, TX, Todd (Davenie) Breeding of Germantown, Sue (Denis) Greenwalt of Chillicothe; great-grandchildren, Madison & Gunnar Greenwalt, and Cole Breeding. A Memorial Service will be held 4:00 PM Sunday, Aug. 28, 2011 at the Farmersville United Church of Christ, 42 S. Broadway St., Farmersville, OH 45325, with Pastor Susan Hartman officiating.
